According to media reports the probe committee of local Gorakhpur administration consisting Additional District Magistrate city, City Magistrate, Additional Director Health and Chief Medical Officer, made detailed comments on purchase and refilling of Oxygen in the BRD Medical College.
 
The probe team has found that there was overwriting in the log book related to purchase and re-filling of oxygen cylinders. In its report the Committee has held supplier Pushpa Sales responsible for stopping the supply of essential liquid oxygen.
 
The Committee has raised a question about the absence of the then principal Dr Rajiv Mishra and head of anaesthesia department, Dr Satish Kumar, from the college on August 10 without any information as Dr Kumar was responsible for the maintenance of continuous flow of oxygen to the hospital wards.
 
At least 30 kids died in 48 hours at BRD Medical College on August 10 and 11. Oxygen supply was disrupted on August 10 evening. According to the report, Dr Kumar and chief pharmacist Gajanan Jaiswal wilfully did not maintain the record of purchase of oxygen. The report also say that anomalies in cylinders' record logbook points to financial irregularity.
